Material Order Form

The following material can be requested from Toronto Diabetes Care Connect (TDCC):

• Promotional materials for the public – posters & flyers that can be displayed and given out to encourage people living with or at risk of diabetes to connect with TDCC (through the website or by phone) to find out about and get connected to available programs
• TDCC forms (pads of 50) – Referral Forms and Insulin Order & Prescription Forms are available in pads of 50 which may be useful to those who are using paper-based referral methods when referring individuals to diabetes supports

These materials are usually provided and shipped free of charge although for extremely large orders TDCC may contact you to discuss potential costs that may apply.
