About Toronto Diabetes Care Connect
Toronto Diabetes Care Connect (TDCC) is a program of South Riverdale Community Health Centre (SRCHC) that provides system level support around access to and delivery of diabetes care within the Toronto region.
Since 2013 TDCC has worked collaboratively with healthcare providers and organizations across the Toronto Central Region.

- Operate the Toronto Central Referral Service (TCRS) and the Toronto Diabetes Care Connect website with a quality improvement (QI) lens
- Support the development and/or use of frameworks, tools, and initiatives to support access to and delivery of quality diabetes care
- Support diabetes-related professional development among healthcare providers working in diabetes (i.e. New Hires Program & Endocrinology Sessions)
Toronto Central Referral Service
The Toronto Central Referral Service (TCRS) can easily connect individuals living with or at risk of diabetes to a number of diabetes programs across the Toronto Central Region and in some cases across Ontario. The TCRS is able to connect individuals to the following types of diabetes-related supports:
- Diabetes Education Programs – within Ontario
- programs supporting type 2 diabetes management and prevention (led healthcare providers)
- 17 community-based programs are located in this region including Diabetes Education Community Network of East Toronto (DECNET), located at South Riverdale Community Health Centre.
- Diabetes Eye Screening Program (Teleophthalmology) – within the Toronto region
- yearly diabetes eye screening for people with type 2 diabetes at a number of locations across the city, including South Riverdale Community Health Centre
- Choose Health Self-Management Program – within the Toronto region
- peer-led group workshops that support people around improving their skills around self-management
- Diabetes Prevention Programs – within the Toronto region
- healthcare provider-led group programs that help people learn how they can lower their risk of developing type 2 diabetes
- Special programs are available that focus on Aboriginal, Caribbean, East African, Latin American, and South Asian populations
Anyone can get connected by completing a referral (e.g. person living with or at risk of diabetes, caregiver, community support person, healthcare provider, etc.).
Referrals can be made on-line, through the mail, or over the phone.
